Honest question how likely is it that identical twins get mixed up at some point during their infancy?

I know as children and adults even identical twins have defining characteristics that allow people who know them well to differentiate them.

However, when they are infants and you don't know them well, and there are no obvious physical differences to differentiate them, how do you ensure you never mix them up?

I assume hospitals use wrist bands or something to identify which is which, but once home all bets are off right? How many new parents in a sleep deprived, exhausted mental state, when giving a bath or changing a diaper might have accidentally swapped baby number one and baby number two? And if they did, how likely would they be to ever catch the mistake?

I actually saw the same GYN that delivered me as a patient. Great doctor. He retired last year at 90. Weird to think that in some families he's delivered multiple generations.

And his sons both went into 'the family business' so I'll probably see one of them when I can travel again.
