I have no doubt that her going insane was the intention from the start, but the main problem was the producers got tired of the show and settled for 73 episodes when HBO offered 100. Seasons 7 and 8 were so rushed that spectacle and subverting expectations became more important than character motivations and development.
Yes, there were indications of Dany's hot headedness early on, but it just wasn't developed convincingly once they got past the book material. It is quite obvious GRRM gave them bullet points early on, and when they ran out of source material, it became Game of Bullet Points.
