Cinematographer David Franco told D&D about this blatant mistake, but they told him "it's fine. we just don't see him anymore". Franco says he was "obsessed" with this error. Him and the actors are laughing about this on the season 8 dvd commentary.

Why Cersei didnt just kill everyone right then I have no clue. Especially when she's show that she isnt above doing something like that.
