I would have killed myself long before that moment. Euthanasia may be illegal, but I'm not suffering through that.

Someone confirm this is actually what skeletal cancer looks like, and not just a 4x4 through the back of his head and out the front?

Looks like it was a chondrosarcoma. Chondrosarcoma is a cancerous growth of cartilage. There's little to no new bone growth, it was rather pushed out of the way. If it was an osteosarcoma the tumor would be solid rather than hollow. Since connective tissue decomposes, it leaves the crater it created behind.
