I have a hard time believing no one in Westeros suspected Jon was Rhaegar and Lyanna's child, even without knowledge of a private wedding ceremony and an annulment. Everyone falls over themsleves in shock that the noble Ned Stark would have a bastard. Everyone knew Rhaegar was obsessed with Lyanna. Like... c'mon.

She didn't hate Ned's cheating, or the fact that he cared for his bastard. It's that he did it in public, with her own kids.
