This happened to me once. I had to leave and go to work before doctor came. I informed the front office staff and agreed to set another appointment. They ended up sending me a bill for a missed appointment that I disputed. They refused to engage with me further and eventually sent it to collections.
11:48 - Just left. I sat in the waiting room for around 15 minutes before getting called back. Nurse finally made it in around 10:20, doctor followed suit at 11:00. It's the fact that I had to schedule the appointment a month out that really frustrates me. If I'm going to hate to wait a month just to get in, I'd like to think the appointment time would have some relevance. Might as well be a walk in clinic at this rate.

All you have to do is try to start a phone call, the doctor will walk in the second you try to do something else
