There is no defence for the US health system beyond incoherent rambling about communism. Massively higher private and public expenditure for massively worse outcomes. It is a criminal system.
In Spain, you pay social security each month and get health care. You could technically become self employed in Spain, pay the minimum tax (which is 50 euros) per month to gain access to the health care system, and then get a hip replacement for free (plus the cost of prescriptions, which is highly subsidized).
I have clients who actually do this. They are heading to Barcelona this week for their second round of dental surgery. Husband, wife and child. They will spend about $20k in total for what was quoted at $40k in the USA.
