Those are the suckers, not eyes. There are more around back. They are for attaching to the host's intestinal wall.
The mouth-looking thing at the bottom are the hooks it also uses to hold on to the wall of the gut while it absorbs your food through its skin, growing longer & longer, gaining more and more skin through which to steal your food.
The truth is much more terrifying, the more I think about it.
