She fainted due to exhaustion.

This happened to a girl at a local high school a few years back. She was on the swim team and obviously an excellent swimmer, but went unconscious and drowned doing laps. The coach had left the pool area to grab some coffee and none of the other swimmers saw her slip under. She wasn’t noticed until laps were over and everyone got out of the water.

"I honestly thought I was asleep," Alvarez said after that ordeal. "I started hearing people saying, 'It's going to be OK.' I thought, 'Stop telling me that! I'm trying to sleep.' Then I realized that no, I was still in the pool."

Had a similar experience with getting knocked out in a rugby game. Can't imagine that happening underwater.
