Vanity Fair magazine used to do an annual Hollywood edition with a fold out cover featuring group photos. There were different themes each year (women, men, young, next big thing etc). The issue would have behind the scenes pictures, they were done as group shots not composites. Annie Leibowitz was the usual photographer for that issue.
