Procedural clinician here in a major academic hospital.

There are elements of this picture I imagine are probably staged.

The fact that the stretcher is still there and in that condition is the most glaringly obvious to me.

The moment you transfered the patient to their destination, OR very likely in this case. You should/ would clean the bed. I suppose you could get a clean stretcher and risk another transfer in a trauma like this but I don't think I've ever seen that done because people aren't prioritizing things like that.

Empty blood bags on the floor like many people mentioned...

Maybe the trash bag liner ripped and someone snapped a picture after centering the bloody stretcher they should've cleaned but more plausibly someone wanted a picture for this purpose and made it look worse than it should've.

I've seen a room like this once or twice legitimately, and it was always in July when we had brand new fellows and trainees, and you had too many people wanting and desiring to help without the knowledge or experience to be effective at the execution.

It was always a learning experience for everyone involved but if I'm being completely honest a bit traumatic.

Glad the patient made it. Both times my room had been like this the patient did not.
