Take him to the vet to have him checked out. I had a cat go missing for 5 weeks, he came back very skinny but I delayed going to the vet for a checkup. We almost lost him for good.
When I was a kid, our cat disappeared for a long time. This was about 45 years ago, so I don't remember exactly how long, but it was definitely months. Long enough that we had assumed he had crawled under a bush somewhere and died.
Eventually, the cat returned wearing a collar that had a phone number written inside. Assuming some other family had "adopted" our cat, my mother called the number to let them know that Dutchy was our cat, but thanks for taking care of him.
Turns out, the number was that of a marina. The cat had wandered aboard a training vessel and spent several months as the ship's mascot sailing around the world. They were very happy to hear that he had found his way home and told us how much they loved having him aboard. Apparently as soon as the ship was tied up at the dock, he bolted from the ship, never to be seen (by them) again.
