This thread has made me realise maybe Americans do it differently.
In my house its
Mattress
Mattress topper
Fitted sheet covering those two.
Duvet that has a duvet cover. Duvet cover is washed 1-2 weekly depending on how much I've slept in it.
Done.
Most people i know do this minus the mattress topper

In my house I do: bed frame > box spring > me >mattress > honey smoked ham> fitted sheet > no top sheet> brioche bun
