Doctor is and always was an academic title, not a medical title. In almost any country, people who want to be called doctor, have to write a separate thesis. Very often 4-6 years of doing nothing but research on top of their Bachelor‘s/Master‘s degree.
Just because people are used to calling their medical professionals "Doctor" doesn’t mean that a real Doctor is worth less. Often it’s quite the opposite. That’s why it’s called MD and not simply Dr.
Isn’t Shapiro a Republican? Surely he isn’t expecting FREE medical treatment just because a doctor happens to be present? Sounds pretty socialist to me.
