Here in the UK that's exactly what happens, they are literally taking the old PM's gear out the back door as the new one walks in the front.
It's harsh but it means there's no dead period and let's the new leader get on with their job.

I’m assuming it’s a holdover from a time when votes had to be counted by hand and it took a long time. But it does seem weird. It’s like awkwardly living with your ex for another few months because the lease isn’t up yet lol
