After informing them of his very severe dairy allergy, the waitstaff assured him that the grilled chicken he ordered was dairy free, despite the marinade containing buttermilk. Less than an hour after eating, the teen collapsed, and later died at a nearby hospital. It was on the day of his 18th bday

I used to work in a pizzeria as a manager and we would have people come in all the time with allergies. I was terrified of causing something like this so we took extreme precautions. After a while we had a bit of a reputation of being a restaurant that actually took people seriously. It was so nice to be able to meet all these different people who were just so grateful to be able to eat out without fear.

As someone with a very bad dairy allergy, this terrifies me.

It's a story from the UK, the staff knew the allergens and dairy wasn't listed, this includes from the kitchen staff as the meat came prepared from another company before cooking. Buttermilk wasn't mentioned in the ingredients at all and was the fault of the manufacturer. We have pretty tight laws on these but somehow manufacturers/processing plants have not disclosed possible allergens in their products for a few high-profile cases.
