In the hospitals I’ve worked in patients who don’t have insurance or a means to pay apply for something called “charity care” where a lot of the medical bills will be at least partially covered. I believe most hospitals have this benefit for lower income patients. Some of these hospitals won’t necessarily be glamorous because they are typically in lower income areas but if they have residency training programs the quality of care will be up to date and high quality. Call around and see.

Since you mentioned you're willing to pay upfront, check out other countries. If I were in your position, I'd go to Thailand's Bumrungrad. They supposedly have comprehensive billing and tons of international patients. I would not trust American estimated billing for something this serious lol they say one price before the procedure, and then charge another, and you're going to have to fight to get anything revised.
