I always rephrase when someone asks me to repeat myself. Saying the exact same words again rarely gets the message across. It's not that they didn't hear you. They didn't understand you.
On the flip side, if you heard what someone said but you didn’t quite understand what they meant, say something like “please elaborate” or “I don’t understand” rather than “what?”

My 14 year old, when I ask him about his homework.
"Hey, bud, where are you at on those history notes?"
"That's over now."
"What do you mean, 'that's over now'? Did you finish them, or can you no longer turn them in because they're too late? Or what? What do you mean?"
"That's. Over. Now."
"Go to your fucking room."
