Actually it’s implied that there’s a lot of people on Earth at the time. Because God puts a mark on Cain so that no one will kill him.

The Bible says that Cain was a farmer and Abel was a shepherd - a "keeper of flocks." I like to think that Cain's remark had an element of irony to it; "am I my brother's keeper, as he is the keeper of sheep? Is he a sheep to me?"
