I didn’t even realize you could order a tortoise like that. I assumed you could do that with animals like spiders and various other bugs. I’m not sure how I feel about the though of a tortoise having a similar experience.
Cold-blooded response: Don't pay for any extras, and if it arrives dead, ship it back as DOA and ask for a replacement.
Warm-blooded response: They should guarantee a live animal on arrival, so the extras are likely just bait. It will probably have what it needs to arrive safely, by default. They just don't tell you that.
